The My-Medic-Notes™ Student Program is all about advance planning for health emergencies. The information recorded in My-Medic-Notes™ lets caregivers, teachers, emergency responders and hospital personnel know during an emergency about vital medical information and personal wishes.

In a medical emergency, minutes can make a critical difference, and My-Medic-Notes™ can communicate when illness prevents the person from advocating on their own behalf.

While everyone can benefit by letting others know who their emergency contacts are and other personal identification details, some students are at higher -risk. For example some children live daily with epilepsy, diabetes, and anaphylactic reactions to bee stings or peanuts. Others need to know what is the first response protocol to help them through their medical emergency, as well as details concerning their medical background.

In the Members-Only section you can access printable forms to create an Emergency Response Action Plan for specific conditions such as Epilepsy, Diabetes, or Anaphylactic Reactions. You'll also find 'quick links' where you can get additional information on first-response-protocols.

We offer 3 different versions of Student Kits - a Home-Pak, a School-Pak, and a Dual-Pouch Kit.

My-MEDIC-Notes™ was developed by the Wellness Programs Division of Health Assured Financial Group by Jacqueline Figas CLU, Director of Marketing and Benefits Planning and Donna Ritch RN, Director of Health Services. We gratefully acknowledge the valuable input provided by Epilepsy Durham Region in the development of the students version of My-Medic-Notes™ and the first-hand experiences related to us by over 100 paramedics that further inspired and shaped the development of this program.
