“My-MEDIC-Notes™" is all about planning in advance for a medical emergency. The information recorded in
My-MEDIC-Notes™ lets first responders know during an emergency about the person’s vital medical information, key contacts, and personal wishes.
It will also provide caregivers to children or older adults with special instructions they need to know so they too can quickly respond with appropriate care during a medical emergency. What they do in the first few minutes could make the difference between life and death.
My-MEDIC-Notes™ is also a convenient way to provide hospital personnel with essential details that will help them to respond faster with the most appropriate medical treatment according to your medical wishes.
In a stressful situation, it is difficult to think clearly and My-MEDIC-Notes™ can do the communicating for you when you cannot advocate on your own behalf.
We would like to acknowledge the excellent work and vital services performed by all paramedics, firemen, first responders, and caregivers ... where would we be without them!

Who is it For?
This program will be of interest to people of all ages. While everyone can benefit from advance planning for a medical emergency, some groups are at greater risk .
For children - when your children are in someone else’s care, it can provide their caregiver with vital medical information in the event a medical emergency should arise. Whether they are at school, with a caregiver, or participating in recreational activities, this information could save their life! Many children today live daily with a medical condition that could be life threatening - epilepsy, allergies to peanuts or bee stings, diabetes. It is critical to provide a medical emergency plan to their teachers and others who may be responsible for their supervision when they are not in your care.
For university students - it can communicate serious allergies, special medical conditions such as epilepsy, and other vital medical information when they are living on their own away from home, if a medical emergency occurs.
If you live alone - it can be your voice when you cannot communicate your medical issues or your wishes to others. Many women today are living on their own - divorce rates are up, women live longer than men ... planning for medical emergencies is a must.
If you are an older adult - it could save your life! And it will give your children who may be providing caregiving assistance at a distance, peace of mind.
If you have a medical condition -it can inform paramedics of your vital medical background.
If you are driving a car - and you are in an accident, a copy of your My-MEDIC-Notes™ placed in your glove box can give paramedics critical medical and contact details if your injury prevents you from speaking for yourself.
